Remote patient monitoring is the use of digital devices to monitor and capture medical data from patients and electronically transmit this information to our care team for assessment and, when necessary, recommendations and instructions.  Common devices include blood pressure monitors, glucose meters, blood oxygen monitors, weight scales, thermometers and more.  For more information, speak to your primary care provider.
